How to make a referral
Please note that the child you're referring must be at least 5 years old.
- Please complete all sections including Referrer Information, if you do not have the information please contact parent/carers before making the referral to obtain the details to ensure there is no delay in the registration process. Without this information, we are unable to process your request.
- By sending this referral you confirm that both the family and the young carer involved are aware of this referral being made and are happy to be contacted.
- Upon receipt of this referral, the young carer will be inputted directly into our system and a welcome letter and consent form will be sent out to the family. It is then the parent/carer responsibility to send the consent form back to us to activate the eligibility to access our activities.
Young Carers’ Needs Assessment
- All young carers aged between 5-18 have a right to an assessment regardless of who they care for, what type of care they provide or how often they provide it.
- A young carer has the right to an assessment based on the appearance of need – which means that young carers will no longer have to request an assessment or be undertaking a ‘regular and substantial’ amount of care.
- A young carer does not need to have an assessment to access any of our services.
Consent MUST be given by both the young carer and a parent/carer to be referred for an assessment and they MUST be made aware that the completed assessment will be sent to the Local Authority to input onto their systems.
