Salary: £20,000 per annum.
Hours: 3 on 3 off (1x 12-hour shift 2x 13-hour shifts)
Purpose of the role:
To provide support and assistance to carers and people with care needs during periods of planned and unplanned absence to ensure the level of support to the service user continues unaffected.
To staff Carers Trust Heart of England services across Coventry and Rugby This will involve liaising with clients, staff and professionals. To respect confidentiality at all times within an equal opportunity’s framework.
PRINCIPAL DUTIES AND RESPONSIBILITIES
·To be flexible and respond quickly at short notice, to emergencies, unplanned or planned absence such as sickness and annual leave and provide a service for carers across Coventry and Rugby.
·To provide practical and emotional support.
·To staff Carers Trust Heart of England out of hours on call service across all geographical locations on a roster basis, managing calls from clients, staff and other stakeholders and responding appropriately.
·To monitor Carer Trust Heart of England electronic call monitoring system out of hours and respond to alerts.
·To liaise with the Project Managers and assist in the office with duties connected with case management including, but not limited to, roistering, assessments and reviews, spot checks and supervisions.
·To undertake the tasks specified in the support plan which may include the following personal and social care tasks:
·Urgent /essential shopping, including urgent prescriptions
·Assistance with activities of daily living
·Support with medication.
·Work within the recommended Health and Safety standards.
·Ensuring that confidentiality is protected at all times.
·Work as part of a team, liaising with colleagues, other professionals and involved family members to ensure the best possible outcome for patients.
·Making individual contributions in staff meetings, supervisions, observations and reviews, to improve patient care and health/wellbeing.
·Producing and maintaining accurate records
·To undertake other specialised tasks as agreed in the support plan and after appropriate training.
·To treat all carers and people with care needs with respect and sensitivity.
·To respect the personal choice of lifestyles of colleagues, carers and people with care needs.
·At least 1 years’ experience of working within the care sector (Essential)
·Experience of working with a not for profit or Health and Social Care organisation (Desirable)
·Understanding of the needs of carers and those they care for (Essential)
·Understanding of the importance of confidentiality (Essential)
·Able to undertake a range of personal care tasks and light domestic duties (Essential)
·Ability to work on own initiative and in a team. (Essential)
·Competent IT skills (Essential)
·Willingness to undertake any necessary training (Essential)
·Ability to work in a flexible manner on a shift pattern (Essential)
